- OtherActive Ageing Centres Team (AAC) Team, Primary Care & Community Care Development Division (PCDD) supports MOH in the management and development of Active Ageing Centres (AACs). The Programme is intended for the centre to be the go-to point for the seniors for their social and health matters. As a member of the team, your key roles will include the following:
**Programme Management**
- Oversee the management of the performance of existing AACs to meet Active Ageing Centres (AAC) service model key performance indicators (KPIs) and service requirements (SRs)
- Oversee service management to support providers in understanding and meeting their current Service Guidelines and reporting requirements, and develop plans to smoothen their transition to the new service model
- Develop strategies to support AACs for training and capability building opportunities to build their readiness for AAC onboarding and ramp up the delivery of the new service model
**Service Development**
- Gain an understanding of AACs’ operations with regular check-ins to build relationships, guide and prepare them in the change management to adopt service expansion
- Organise cross-learning platform to support AACs in meeting the project KPIs and sharing of best practices in preparation of the transition and implementation of the AAC service model
- Drive service development and quality improvements to meet the changing needs of the evolving eldercare landscape
- Work closely with stakeholders to troubleshoot issues faced and support AACs to address identified gaps and improve efficiency of their processes
- Support the new developmental efforts and continuous review of the model to meet the changing needs and evolving eldercare in line with Age Well SG
**Job Requirements**:
- At least 5 years of working experience in service planning and development, with at least 1 year in managerial capacity in a leadership role, preferably in the healthcare or social care industry
- Experience in policy formulation, planning, strategy development, sector development or project management in the public sector would be advantageous
- Strong at conceptualising ideas, coupled with a practical sense of what works in a complex environment
- Possess strong analytical, critical thinking and problem-solving skills with a good eye for detail and strong follow-up skills
- Possess strong organizations skills and effective project management skills in managing multiple projects concurrently
- Strong ability to handle ambiguity in a fast-paced work environment
- Able to demonstrate strong interpersonal skills in handling diverse situations
