
Programme Executive/assistant

**Primary Objectives of Position**:
- To implement outreach plans to the seniors living in our neighbourhood, to invite residents to be AWWA members to improve social participation and ensure timely coordination of care based on their individualized profile & needs.
- To conduct home visits and call check-ins to maintain Buddy and Befriending services, and to conduct engagements for Buddy & Befriending clients.
- To build and maintain a pool of befriending volunteers, to promote the spirit of volunteerism among seniors and to establish a supportive environment, and culture of contributing meaningfully back to the community.
**Responsibilities**:
- Support the planning, coordination, and execution of engagement which includes a variety of health & social activities to engage active participation/volunteerism from seniors.
- Profile participating seniors and ensure seamless holistic care for them, based on their health and social needs.
- Maintain proper documentation of participation records and seniors’ profiles for reporting purposes. Access and update client’s records on regular basis to document client’s particulars/progress/interventions and/or to review/update care/engagement plans.
- Perform administrative duties such as attending to information & referrals, monthly update of members’ data, maintaining of expenditures, inventory of office equipment and furniture, recording fund expenditures, monthly rations, and directory of social service agencies in the community, if any.
- May need to offer direct care services to the clients, when required.
- Manage and respond to the emergency Alert Alarm System at Blk 123
- Adhere to professional standards in legal, ethical, health, and safety practices.
- Ensure clients’ confidentiality is maintained and adhered to PDPA policy when sharing client details with approved stakeholders, and using systems including and not limited to: the Aurora database system, IRMS, CCTV, and Visitor Management Record system.
- Adhere to the Centre’s SOP and service requirements at all times.
- Collaborate with Corporate Affairs Department and contribute with content in relation to AAC and to the handicraft project.
- Understand MISSION Framework, and use relevant tools to support the Professional Practice of self and the team members.
**Other Responsibilities**
- Contribute to AWWA’s projects and committees as and when required, which aligns with organisation’s strategies and mission.
