DESIGNATION : Social Work Associate (Home Care)

RESPONSIBILITIES

**Patient and Family Support**:

- Assist in assessing the psychosocial needs of patients and their families to ensure appropriate care and support.
- Provide emotional support to patients and families as they navigate the challenges of illness, disability, or aging.
- Facilitate discussions with patients and families to address concerns and set realistic expectations.
- Promote and educate effective caregiving to caregivers.
- Support in developing and running caregiver programmes.

**Financial Assistance**

**Care Coordination**:

- Help coordinate care services, including arranging for home health aides, medical equipment, and necessary follow-up care.
- Monitor the implementation of care plans and make adjustments as necessary to improve patient outcomes.

**Resource Navigation**:

- Identify and connect patients and families with community resources, such as meal programs, transportation, financial assistance, and caregiver support.
- Provide education on available resources and advocate for patient access to these services.

**Urgent Case Intervention**:

- Identify cases with urgent care needs or attention, example abuse, suicide and high caregiver burden.
- Flag out urgent cases to Medical Social Worker.
- Work closely with supervisors and healthcare teams to respond promptly to crisis situations, ensuring patient safety and well-being.

**Documentation and Reporting**:

- Maintain accurate and timely records of patient interactions, care plans, and interventions in accordance with organizational policies and procedures.
- Prepare reports on patient progress, resource utilization, and other relevant data for review by supervisors and healthcare teams.

**Professional Development**:

- Participate in ongoing training and professional development opportunities to stay current with best practices in social work and home care.
- Attend supervision sessions and team meetings to review cases, receive feedback, and enhance service delivery.

Any other related duties as directed by the said reporting officers.

QUALIFICATIONS
- Minimum Diploma in social service or equivalent.
- Able to work with senior group and converse in vernacular language.
- Prior experience in social service &/or healthcare setting preferred.
- Possess good interpersonal and communication skills; demonstrate understanding and listening skills.
- Good time management skills, well-organized, efficient and able to multi-task.
- Takes initiative and able to work well independently and as a teamplayer.
