**STATEMENT OF PURPOSE**:
The Case Management Officer supports and manages the patient’s overall recovery and works closely with clinicians in providing appropriate services and treatment. The Case Manager plays an integral role in the multi-disciplinary team; they act as a bridge between patients and psychiatrists, also helping patients transit back to their community and family setting.

**DEPARTMENT FUNCTION(S)**

**Major Duties and Responsibilities**
- **SPECIFIC (80%)**
- Develop a partnership with patients, assess and anticipate care requirements, make plans and ensure that patients receive appropriate care and services.
- Achieve anticipated clinical outcomes and involve the care team by providing information and updates on patients’ treatment progress.
- To familiarize with the community resources, initiate and coordinate referrals between patient and community partners.
- Perform data management, collating and maintaining of database for clinical and project submission.
- Adhere to professional standards and integrity as outlined by protocols, rules and regulations.
- Document all case information, forms and reports accurately
- Support the department in development and goal achievement

Specific Areas of Responsibility
- Accomplishes patients’ care by assessing treatment needs, developing individualized care plan, monitoring and evaluating treatment plans and progress.
- Identify, analyze and find the causes and make adjustment to the patient’s care plans and goals which have not been achieved.
- Educate and provide adequate information to ensure patients understand their condition/s and treatment needs.
- Facilitate/Coordinate interdisciplinary meeting and treatment plan for patients’ recovery.
- Coordinate the care delivered to the patients and facilitate communications between care providers and patients.
- Update multi-disciplinary team on patients’ progress and matters that affects patients’ recovery.
- Implement any required corrective measures to the clinical treatment plan with the multi-disciplinary team.
- Facilitate the plans for patients’ discharge with the Multi-disciplinary team and coordinate the plan with patients.
- Promote and enhance the development goals, objectives and systems to support the department’s objective and goals.

**JOB REQUIREMENTS**:

- EDUCATION, TRAINING AND EXPERIENCE
- Knowledge - Mental Health, Behavior science, suicide assessment and intervention, case management principles, familiarity with professional and technical emerging knowledge, community resources and governmental policies that impact the patients and basic counselling techniques.
- Skills - good interpersonal communication, advocacy, problem solving and administrative duties.
- Communication skills: to be able to communicate effectively and listen actively. Case managers must be also be aware of nonverbal communication cues and be able to read nonverbal cues with patients.
- Interviewing skills: to be able to evaluate the patients’ needs. Case managers need to be able to formulate questions, record client information and answer patients or family members’ questions promptly.
- Teaching skills: to be able to help patients develop skills and knowledge to improve their life situation. Case managers need to help patients develop social skills, regain independence, identify and build coping strategies etc.
- Advocacy Skills: case managers have to engage in advocacy on behalf of their patient. Case managers may attempt to resolve conflicts between patients and services providers and work towards protecting the best interests of the patients.
- Organisational skills: Case managers have to be adept at planning and organisation to ensure service delivery to their patients in a timely manner.
- Aptitude for teamwork and ability to work well with diverse groups, from other professionals to patients’ family members. Case managers don’t work alone to help their patients improve their quality of life. Instead they help patient achieve their goals as par
