**Job Summary**

Lead and guide team to work with Caregivers and families, implement and evaluate program and engage stakeholders
- Scope of Caregivers Support: Provide support to caregivers and families of older persons, person with special needs and mental health conditions.

**Main Responsibilities**
- 1) Direct engagement with Caregivers to empathsize with the caregiving journey:

- Perform functions of a Care Coordinator for TOUCH Care Line.
- Face to face engagement with Caregivers and family members during programs, services and workshops.
- Virtual engagement with Caregivers via social media platforms e.g. facebook and telegram

2) Program design and evaluation:

- Contribute to analysis of the landscape to design program and services
- Analyse data collected from program and services.
- Assess and evaluate the health and social outcomes of programmes.
- Analyse and review gaps to formulate programmes to meet various organizational goals.
- Plan and implement programmes or workshops (face to face, virtual meetings or on social media platforms) catered for Caregivers.
- Review capability of the team regularly with Head to upkeep professionalism and prevent burnout in the team.
- Establish and maintain program timelines and priorities
- Support Head to identify and resolve operational issues, complaints and feedbacks.

3) Stakeholders management:

- Establish and maintain relationship with community and internal stakeholders to deliver suitable programmes for the community.
- Plan and collaborate with other departments to achieve organisational goals.
- Liaise with corporate and school volunteers to plan and implement ad-hoc events or regular activites for Caregivers.

**Working Conditions**
- 5-day work week: Mon - Fri, 9am - 6pm
- Attractive remuneration and staff benefits

**Requirements**:

- Degree in any discipline
- Interested to improve Caregiving situation in Singapore
- 3-5 years experience in working with Caregivers and Families
- Experience in working with Family
- Experience in resources and respite services for Caregivers
- Social work and counselling experience is a bonus
- Independent worker and strong team player
- Creative and adaptable to complex situations and volatile environment
- Good interpersonal and communication skill
- Experienced in stakeholder management
- Strong organization and planning skill
- Strong critical thinking and problem-solving skill
- MS Office 365
