Head Investigation & Safety Audit
**Job Purpose**
- 1. Specialist to conduct investigations on significant workplace incidents and near misses.
2. Specialist to advice BUs in matters relating to investigation standards, methodologies, root cause analysis
3. Conduct analysis of past incidents to ascertain trends of concerns and suggest areas for systemic improvement.
4. Plan and implement independent inspection programme to validate ground compliances and determine the effectiveness of controls within the Risks and HSE Management System in meeting organization objectives; Recommend initiatives for continual improvement.
5. Report outcomes of audits, inspections, and investigations to Management & Board.
6. Represents SMRT and be the liaison and coordinates with regulators and external agencies (eg MOM, TSIB, LTA) for the purpose of incident investigations
**Responsibilities**
- Duties of Head Investigation & Safety Audit are listed below. Note that the list is not exhaustive and related duties and responsibilities may be assigned from time to time.
- 1. Ensure all BUs adopt the approved investigation methodology and adhere to the investigation directive in SMRT.
2. Check and ensure a high quality standard of all investigation reports from BUs, and that all incidents are able to establish the root causes and appropriate follow up recommendations.
3. Maintain a register of all investigation reports in SMRT
4. Implement a tracking system to ensure competency of all Investigation Officers in SMRT are adequately qualified
5. Plan and manage the risk-based audit and inspection program with focus on safety, security and quality (SSQ) matters of the employees, stakeholders, as well as system within the respective business units in SMRT
6. Collaborate with Internal Audit Department (IAD) and other Business unit SSQ teams to plan and schedule audits and inspections based on the approved annual audit and inspection plan.
7. Validate ground compliance to established authorized operating document, past incident/ audit/ inspection findings, KRIs and risk registers.
8. Advisor to Business Units (BU) on SSQ matters, including emergency preparedness
9. Perform Investigation as directed by Board, GCEO and/or CSSO, where required
10. Prepare, issue and file audit, inspection, and investigation reports and supporting documents
11. Maintain Corp SSQ documents in compliance to regulatory and industry standards and best practices
12. Trend and provide regular updates to management on outcomes of audits, inspections, and investigations based on the scope or area assigned to him/ her.
13. Track and monitor corrective actions arising from audits, inspections, and investigations.
14. Main SMRT representative with relevant regulators and authorities to coordinate on incident investigations.
15. Assist CSSO to provide regular investigation trend status to safety meetings (eg SSQC, MCM, BSSC, Board Meetings)
**Qualifications & Work Experience**
- Qualifications and work experience include:
- 1. BSc degree in Engineering or Science (Environmental, Occupational Safety and Health Occupational Safety and Health)
2. Registered Workplace Safety & Health Officer (WSHO) and Fire Safety Manager (FSM)
3. Qualified IRCA lead ISO45001 /ISO14001/ISO9001 auditor, with at least 10 years of working experience in conducting internal audits preferred
4. Trained and experienced in investigation methodologies such as 5M or Systemic Cause Analysis Technique, with at least 10 years of investigation experience preferred
5. Qualified in WSQ Graduate Certificate in Workplace Safety and Health (Level D) is highly advantageous
6. Knowledge and experience in rail operations and maintenance is highly advantageous.

3. Proficient in MS Word, Excel and PowerPoint
