We are looking for a dynamic and energetic team-player who is keen to join and grow with our team as a Senior Staff Nurse (Day Care).
Reporting to the Nurse Clinician, the Senior Staff Nurse supports and provides care to patients with advanced life-limiting illnesses.
Job Scope:
- Perform comprehensive palliative care assessment of day hospice patients
- Perform screening & triaging of new referrals
- Provide consultative advice, social engagement & psychosocial support to patients and family members based on care needs.
- Support families and caregivers of patients and work with the rest of the multi-disciplinary team to provide symptom management, nursing care, psychological, emotional, and spiritual care.
- Monitor patient’s symptoms whilst attending day care, administer medications as necessary and conduct appropriate escalation of care
Requirements:
- Diploma or Degree in Nursing
- State Registered Nurse with minimum 5 years of nursing experience in acute care setting.
- 3 years’ experience as a staff nurse in a palliative, geriatric, or oncology care setting and/or community/centre-based setting is preferred.
- Post-Basic training in Palliative Care Nursing, Gerontology or Community Health will be an advantage as it is relevant to the role in terms of skillsets.
- Ability to speak a second language or dialect(s) is an advantage the patients comes from diverse groups.
- Self-directed with the ability to work with little supervision.
