The Job Holder is responsible for supporting the development, implementation and evaluation of strategies and programmes to enhance the integration of care and to improve population health in the NUHS Regional Health System. The Job Holder shall support the smooth running of operations while working with stakeholders and community providers to effect NUHS population health strategy to facilitate the anchoring of care in the community.
Responsibilities include:
- Support the development, implementation and refinement of the NUHS CareHub’s strategic plans, in line with service planning directions and strategies from NUHS RHSO and MOH.
- Support the development, implementation and operations of programmes and enablers (e.g. IT-related projects) to enhance care integration and improve population health.
- Assist with collating of agenda and presentation materials for meetings to drive development, care integration and implementation.
- Any other duties / projects as assigned.
**Requirements**:
- Basic degree in any discipline.
- Good analytical and organization skills.
- Strong communication, presentation, writing and interpersonal skill.
- Able to execute and operationalise plans well.
- Possess positive attitude.
- Experience in the Singapore healthcare system and/or knowledge of integrated care models will be advantageous.
