Singapore STROKE SUPPORT STATION Full time

Job Scope
- Conduct assessment for clients during admission and periodic reviews
- Coordinate, implement and assist to provide intervention for clients on case plans
- Participate in case discussion with the care team
- Conduct financial assessment and provide financial counselling and assistance
- Ensure timely renewal of means testing and financial assistance
- Consolidate, prepare, track relevant monthly reports and statistics
- To perform any other administrative work required
- Any other duties as assigned by Reporting Officer

**Requirements**:

- Diploma in community services / social work / counselling
- Able to converse in English and other languages
- Minimum 1-2 years of relevant experience in the healthcare sector.
