The **Rehabilitation Therapist** will be responsible for leading and implementing rehabilitation programs at Apex Harmony Lodge. Duties must be carried out in adherence to professional ethics, statutory regulations, departmental standards, and institutional values. The incumbent is expected to fully support the Lodge’s mission, vision, and values, demonstrate sound knowledge of healthcare systems, and effectively collaborate with a diverse workforce to enhance residents’ quality of life.

The incumbent will provide leadership and guidance to staff in delivering comprehensive rehabilitation services and programs, while actively participating in departmental and institutional initiatives that contribute to holistic resident care.

**Key Responsibilities**

**Assessment and Evaluation**
- Conduct thorough assessments of residents’ physical, emotional, and social needs to develop individualized treatment plans aligned with therapeutic goals.

**Treatment Planning**
- Design and implement rehabilitation programs, including physical exercises, therapeutic activities, and interventions to improve functional abilities and overall well-being.

**Therapeutic Interventions**

**Progress Monitoring**
- Track and document residents’ progress, review and adjust treatment plans as necessary to ensure effective outcomes.

**Education**
- Educate residents and staff on exercise regimes, therapeutic techniques, and wellness practices to support functional, psychosocial, and emotional well-being.

**Collaboration**
- Work closely within multidisciplinary team, which include physiotherapist aides, exercise therapists, psychologists, nurses and other healthcare professionals to deliver integrated care and achieve best rehabilitation outcomes for residents.

**Engagement**
- Actively participate and contribute in departmental & institutional overaching programs, events, committees, and workgroups etc.

**Administrative Duties**
- Maintain accurate resident records, schedule staff members’ duties equitably, prepare required reports, and ensure compliance with healthcare regulations (e.g., InterRAI, Healthcare Service Act).

**Job Requirements**
- Bachelor’s degree in Physiotherapy or Occupational Therapy (or equivalent).
- 3 - 5 years of professional experience as a Physiotherapist or Occupational Therapist.
- Registered with the Allied Health Professions Council and holder of a valid Practising Certificate (PC).
- Experience in eldercare and dementia care, with the ability to integrate evidence-based interventions into nursing practice.
- Prior experience in teaching, curriculum development, or capability building in nursing will be advantageous.

Pay: $5,000.00 - $7,000.00 per month
