The Social Worker plays a professional role in the field of social work practice. He or She is responsible for the delivery of direct care of service to clients with mental health issues, including assessment, casework management,counselling, psychoeducation and caregiver support, under the supervision of the Senior Allied Health Professional. To ensure a holistic care approach, he or she may also be required to collaborate with other community agencies,based on the needs of the clients
- Conduct assessment and provide intervention and support to clients with mental health issues and dementia, eg.psychoeducation, case management and counselling, as well as caregiver support and training.
- Carry a caseload of clients under Casework and Counselling
- Provide comprehensive mental health assessment, case management and psychosocial interventions
- Assist in the planning, implementation, monitoring and evaluation of group work and provide recommendationsfor consideration
- Responsible for maintaining client records that are accurate and timely for submission
- Support in the development of COMIT service planning document and manual including outreach, screening,mental health information, service linkage and follow up
- Assist with company projects or report submissions, as assigned by supervisor
- Network with stakeholders
- Be available for work and urgent cases during off hours
- Carry out any other duties as assigned by HOD or supervisor

**Technical Competencies**:

- Carry out any other duties as assigned by HOD or supervisor
- 2 years experience in relevant fi eld of social work practice, especially casework and counselling, group work and community work

**Core Competencies**:

- Proficient in oral and written communication, especially English
- Demonstrate sound and updated professional knowledge and skills in social work
- Good analytical skills

**Preferred achievements / characteristics**:

- Good interpersonal and teamwork skills
