Brahm Centre is a registered mental health charity dedicated to promoting happier and healthier living through holistic programs. We empower individuals to create their own happiness. We are a full member of the National Council of Social Services and an Institution of Public Character.

We offer science-based mindfulness programs to enhance mental well-being.

We have a care management team supported by trained volunteers. Working with the Agency for Integrated Care, the Ministry of Health and healthcare and educational institutions, we support the community through health education, case management, counselling services and outreach. We offer wellness programs and art courses designed to enhance overall well-being.

**Job Description**:
The AAC program facilitator will be responsible for the planning, coordinating and delivering engaging and meaningful activities for seniors. He/She will also be responsible to monitor and work with the team to drive towards attainment of AAC KPIs.

Key responsibilities include:

- Program planning and delivery for age and frailty appropriate programs as per AAC requirement.
- Collate feedback and review the programs regularly with the team to ensure that the programs address the various interests in the community.
- Foster positive relationships with seniors and ensure regular attendance and participation of AAP.
- Assist in promoting centre activities and registration and keep the community updated on the programs available in the centre.
- Maintain accurate attendance records in the system.
- Ensure all activities are conducted in a safe manner.

**Requirements**:

- Good Customer Service with excellent interpersonal, written and oral communication skills
- Passionate about serving seniors in the community
- Good attention to details
- Ability to speak Chinese and dialects
- Ability to organise and host workshops and events is an advantage
