**A. Programme Development and Implementation**
1. Plan, implement and monitor the outreach, care process, referral and follow-up workflow.
2. Provide supervision to the team to provide care and support, in alignment with the overall CREST programme objectives, adhering to the relevant ethical and legal guidelines.
3. Provide care coordination and service linkage to clients with or who are at risk of mental health issues with the appropriate resources and support including caregiver support.
4. Responsible for establishing the network with other stakeholders like community partners and/or related agencies to build collaborative partnerships and better facilitate care and support for clients and caregivers.
5. Establish and review work and communications workflow including identification, referral and escalation work processes as well as the development/update of the CREST service manual.
6. Any other duties as assigned by the Head, SACS Psychiatric Services.
**B. Administration and Staff Development**
1. In charge of the analysis and review of all collated data and monitoring of the team’s progress and performance.
2. Responsible for ensuring that the claims and data submission to AIC is timely and accurate.
3. Ensure programme data and documents are kept and maintained, in accordance to the CREST programme requirement, as well as the prevailing personal data protection and security guidelines.
4. Oversee the training and professional development of the CREST staff team with relevant competencies to provide the appropriate care and support for clients and caregivers, aligning to the intended care model of CREST. This includes ensuring all new/existing staffs are clear on their job scope and responsibilities.
5. Ensure that the team practices are in adherence to the prevailing organisational, ethical, safety and legal requirements, where relevant.
**C. Other Duties**
Participate in relevant meetings with key stakeholders including AIC to share key learning
points where required.
