Social Work Associate

The Social Worker works with service clients of the cluster support. He or She is responsible for facilitating change and improving the lives of seniors and their families through coordination and provisions of direct services.
- Able to manage 30-40 cases.
- Conduct Home Visits for the purpose of casework management, gathering information, conducting psycho-social assessment, identifying common needs and goals for intervention.
- Support case conferences with partners.
- Proper documentation of cases.
- Ensure reports on cases are submitted in a timely manner.
- Attend staff meetings, case consultations, supervisions as required.
- Any other ad-hoc duties as directed by supervisor or Cluster Director.
**Job Requirements**
- Minimum Social Services related Diploma or WSQ Diploma in Social Service.
- Preferred at least 2 years of social work setting experience.
- Keenness in doing eldercare case management work.
- Good communication, interpersonal skills, Team player.
- Attention to details and ability to deal with sensitive and confidential information.
- Able to converse in dialect preferred.

**Centre Location**:Beach Road
Peace-Connect Cluster Operator (PeCCO) is a voluntary welfare organisation appointed by the Ministry of Health (MOH) to operate a cluster of programmes for the senior citizens living in the Kampong Glam.
PeCCO provides social support to seniors with little or no family support and assist them to age-in-place within the community through case management, counselling and monitoring their well-being throught home visitations.
