Care Coordinator

We are looking for Care Coordinator to ensure Osteoporosis (OP) patient care delivery is carried out according to established standards of care and practice using patient care plan and clinical pathway.
**Responsibilities**
- Identify patients who have had a previous fragility fracture using ICD codes, from patients attending clinics or A&E, or admitted to Ng Teng Fong General Hospital.
- Review electronic medical record, select and recruit patients who fulfill the inclusion criteria to participate in the OPTIMAL program.
- Recommend investigations relevant to the evaluation of osteoporosis to referring team
- Conduct patient education/ counselling sessions on osteoporosis
- Coordinate bone mineral density (BMD) and doctor appointments at the polyclinics and hospitals.
- Track progress of patients through phone contacts and during clinic visits at regular intervals for a period of time and assess medication adherence.
- Assume responsibility for data collection and entry into OP database system.
- Submit compiled OPTIMAL data and report if required
- Collaborate and liaise with OPTIMAL team members on the care and follow-up of osteoporosis patients.
- Participate in interdisciplinary care team meetings and assist in developing and reviewing care plans, admissions and discharges of osteoporosis patients.
- Participate in audits, QA activities and projects as assigned by the Program Director.
**Requirements**:
- Registered Nurse with a minimum of 2 years’ post-registration experience
- At least 2 years of clinical experience in an acute care hospital would be preferred
- Good interpersonal and communication skills and able to demonstrate a considerable degree of tact and discretion in handling people and situations.
- Able to work independently and effectively under pressure while maintaining a high standard of work in all areas
- Good IT capabilities, able to use electronic medical record
