1. Oversee comprehensive case management for all renal patients supported by the foundation, including referral, intake screening, home visits, risk assessment, care planning, and discharge.

4. Conduct means testing and perform social and financial assessments of patients.

5. Collaborate with a multi-disciplinary team including volunteer engagement to ensure holistic care management.

6. Ensure the psycho-social and emotional well-being of patients is effectively addressed and supported.

8. Maintain accurate documentation and reporting of casework.

9. Develop, review, and update Standard Operating Procedures (SOPs) for Social Service.

10. Prepare periodic reports on social services, including patient demographics, service utilization, and program participation.

11. Contribute to strategic planning for Tzu Chi’s renal care initiatives, offering insights and recommendations based on frontline experience and social work expertise

12. To co-develop, design, plan, facilitate, monitor, evaluate programmes for patient and outreach programmes.

**Experienced Required**

2 to 3 years

**Minimum Qualification**

Bachelor Degree, Graduate Diploma or Diploma in Social Work

**Job Requirement**

1. Preferably registered with the Singapore Association of Social Workers

2. Good written and spoken English & Chinese to communicate with non-English speaking volunteers and patients.

3. Good communication, interpersonal skills and independent.

4. Empathy, compassion, and a strong commitment to helping the needy.

5. Experience working in dialysis centre will be an added advantage
