Social Worker/case Worker at Mobile Support Team

SAMH manages the Mobile Support Team (MST) is a community based, allied health-led team which provides psycho-social therapeutic interventions for individuals with mental health needs and support for their caregivers to maintain clients wellbeing in the community.
**Key Responsibilities**
Responsible for the provision of case management services and support for persons in recovery from mental health conditions, as well as their caregivers.
- Provide case management support and develop intervention plan.
- Deliver rehabilitative services to promote clients' functional independence.
- Outreach Involvements.
- Conduct home visits as required.Other administrative and support duties assigned.
**Qualifications & Requirements**
- Degree in Psychology, Social Work, Counselling, or a related discipline.
- Prior experience in case management and/or the mental health sector is preferred.
- Self-motivated, proactive, and able to work independently.
- Strong communication skills and resourcefulness in problem-solving.
- Demonstrates teamwork and collaboration.Experience in the social service sector will be an added advantage.
