This position is open for both full-time and part-time.

The Cancer Support Worker’s core duty is to provide care and support for clients through psycho-social interventions which contribute to the client’s overall well-being and improved quality of life. Reports to the Director of the Serenity department.
- Identify the client’s needs through the biopsychosocial-spiritual assessment
- Carry out close monitoring of clients through follow-up contacts and individual sessions
- Facilitate family sessions as and when the need arises
- Facilitate client’s attendance to support groups
- Encourage clients to attend enrichment programs and psychoeducation sessions
- Recommend clients to receive services and assistance according to client’s needs and eligibility, and or counseling if the need arises
- Arrange for clients to receive visitations by Volunteer Care Officer or spiritual support by representative from a religious institution if the need arises
- Conduct home visits to assess home conditions and check in on clients who have difficulties to travel
- Link clients to relevant resources or services from external agencies for added support
- Document interventions and interactions with clients in case files
- Facilitate support group sessions
- Moderate psychoeducation sessions
- Recommend suitable clients to be featured in outreach programs or campaigns
- Handle assigned administrative or coordinating duties
