Singapore 365 Cancer Prevention Society Full time

**Purpose of Job**:

- Visit cancer patients to provide them and their families with well-rounded support in battling cancer in accordance with 365CPS’ mission and values
- Maintain regular contact with cancer patients to update them on 365CPS’ programmes and services
- Provide administrative support on patient visitation matters

**Key responsibility areas**

Patient Visitation
- Visit clients at their homes or in hospitals to understand their state of emotional and physical health
- Befriend clients and their families
- Accompany clients to medical appointments where required
- Advice clients on other available resources (e.g. medical social workers) where appropriate
- Guide volunteers and acquaint them with clients during visitation

Client Engagement
- Follow up with non-active cases via phone calls or newsletters
- Encourage clients to participate in 365CPS’ programmes and services

Visitation Administrative Support
- Prepare reports on visitation updates
- Maintain database to manage details of clients

**Requirements**:

- Minimum 2 years of experience, preferably in the capacity of a caregiver
- Certificate in Social Works
