**Job Description**:
Job Summary
Renal Coordinator is responsible to be the main point of contact to liaise with various stakeholders (e.g. nephrologists) as well as partners (e.g. GPs, Polyclinics) in the coordination of patient care, where required. The coordinator has to also proactively track patients’ conditions and perform urgency assessment, and refer back to nephrologists for further management, where necessary. He/she should also arrange counselling sessions with patients and family members, providing them education and counselling with regards to the complications, lifestyle interventions and risk factor control to prevent the progression of renal diseases diagnosed. He/she should also support and contribute to the multidisciplinary patient management.

Job Responsibilities

Liaise with various stakeholders (e.g. nephrologists) with regards to patient’s dialysis access management as well as partners (e.g. GPs, Polyclinics) in the coordination of patient care, where required.

Facilitate the appointment for patients with dialysis access issues, coordinating between Department of Renal Medicine, Vascular, Radiology, and the Emergency Department.

Arrange appointment for patient and family members for counselling sessions.

Conducts education and counselling for patients and their families/carers with regards to complications, lifestyle interventions and risk factor control to prevent progression of renal disease conditions.

Develop, plan and implement clinical protocols for the management of renal diseases.

Track patient’s conditions and laboratory results on a regular basis, and arrange or plan for outpatient visits/inpatient admission, where applicable, and perform urgency assessment of dialysis access with the Nephrologist.

Contribute to multidisciplinary patient management.

Create, maintain and update patient’s database.

Ensure that appropriate and complete documentation is performed and maintained.

Provide monthly statistics to supervisors.

Demonstrate knowledge and the ability to monitor and coordinate care of the patients.

Demonstrate proper and economical usage of consumables and supplies.

Maintain a clean and safe working environment.

General

Demonstrate knowledge and ability to monitor and coordinate care of the patient on identified problems, complications and interventions.

Recognise potential problems and/or significant changes in the patient’s condition during follow-up so as to initiate earlier evaluation and management by other healthcare personnel.

Assist with education and counselling of patient’s progression and evaluation of their needs with individualised case management, regarding the disease and its complications.

Ability to work successfully and communicate with referring physicians, other healthcare providers and other organisations regarding the patient’s status and outcome.

Support the renal medicine department in any other projects or departmental activities, where required.

**Requirements**:
Bachelor degree in health sciences or healthcare related (e.g. nursing, social work, counselling, psychology, social work etc.) or nursing recognised by Singapore Nursing Board (SNB).

Fresh graduates are welcome to apply. Prior healthcare experience would be a advantageous.

Patients may be mostly be geriatric. Knowledge and able to converse in basic dialects would be advantageous.

Proficient with MS Office Applications (e.g. excel, PowerPoint, Word, etc.)

Comfortable in handling and picking up new computer skills.

Possess good interpersonal and communication skills
