Wound Clinician

**Responsibilities**:
**Specific**

You will work under the direction and leadership of the Advanced Practice Nurse in the following areas of patient care:

- Utilise the nursing process when directing or delivering care to patients with Wound, Ostomy & Continence.
- Be involved in onsite, virtual wound consultancy projects with our various partnering Nursing Homes
- Utilise and maintain current physical, psychosocial, clinical assessment, and therapeutic communication skills to determine individual patient needs and expected outcomes.
- Provide consultation and assistance to staff in developing and implementing protocols used in the identification and management of patients with potential or actual alteration in skin integrity.
- Provide guidance to staff in implementation of protocols to identify, control, or eliminate etiologic factors for skin breakdown, including selection of appropriate support surfaces.
- Establish protocols and guidelines for appropriate and cost-effective resource allocation.
- Provide appropriate education to patients, caregivers, and staff regarding skin care, wound, ostomy and continence care.
- Provide follow-up for patients with acute and chronic wounds through Nursing Home virtual/phone consultations.
- Initiate appropriate referrals for medical or surgical interventions.
- Analyse stocked items and recommends appropriate additions and deletions to assure the quality and cost-effectiveness of products used for skin and wound care.
- Conduct product evaluations or contributes to research studies related to skin and wound care and submits reports and recommendations based on the results.

**Specialize Care**
- Provide consultation and assistance to staff in developing a plan of care to manage patients with wound, ostomy & continence care.
- Evaluate the patient’s response to treatment and the progress toward wound healing and makes adjustments and modifications in care as indicated.
- With a physician’s order, provides appropriate debridement of devitalized tissue and to control minor bleeding.
- Assess the stoma and stoma functioning and initiates appropriate stoma care.
- Identify risk factors and assess patients with urinary or faecal continence disorders.
- Identify patients requiring referral for assessment/management of complex Wound, Ostomy and Continence disorders.
- Serve as a resource for Nursing Home staff through virtual/telephone consultation.

**Education**
- Participate in implementing procedures and protocols, based on SLH clinical guidelines, to deliver care to patients with the following:

- Urinary or intestinal disorders resulting in the need for an ostomy
- Conditions requiring wound management
- Impaired or altered skin integrity
- Urinary and/or faecal continence issues
- Provide in-services continuing education programmes.
- Keep abreast with current knowledge and competence in Wound, Ostomy and Continence care.
- Assist internal / external staff to maintain current knowledge and competence in the areas of skin and wound care through orientation, in-service programs.

**Quality Improvement**
- Keep abreast of new clinical products available in the market.
- Assist in the evaluation of products and ability to make recommendation for products and equipment related to wound, ostomy & continence care.
- Operate all equipment correctly and safely.
- Lead or be in a team to execute Quality Improvement Projects annually

**General**
- Maintain and submit required activity and statistical reports.
- Assist to source for, maintain and coordinate usage of all training resources.
- Maintain an inventory list of consumables, tools and materials during consultancy visits.
- Perform other duties as assigned.

**Requirements**:

- Registered with Singapore Nursing Board.
- Diploma/Bachelor Degree in Nursing with a minimum of 2 years of working experience as Assistant Nurse Clinician in wound management.
- Possess a certification in tissue viability or wound care would be an advantage.
- Able to work independently when needed and collaboratively with others
