**Job description**
- The Social Work Associate - Case Management works closely with Agency for Integrated Care (AIC) on referrals from restructured hospitals and other voluntary welfare organizations (VWOs) to provide high quality care for the residents and their families. The incumbent shall provide administrative and programmes support to the Case Management team, in supporting and ensuring a co-ordinated approach with multi-disciplinary stakeholders to review regularly aspects of medical, nursing, allied health and social services concerning residents for the purpose of care management.
- Referral & Admission
- Assessment of all referrals for admission / rejection according to criteria, and flag up any outstanding issues with Medical Social Worker / Senior Medical Social Worker for discussion
- Assists in consolidating and providing updated information for referral assessment as requested
- Works with and support Senior Executive in preparation of admission documents
- Supports Medical Social Worker / Senior Medical Social Worker during resident’s admission, in following up with processing activation of financial payout schemes
- Psychosocial
- Assists in simple case management of residents with non-complex family dynamics or social issues
- Works with Community and Partnerships and Allied Health to develop initiatives for resident/family engagement
- Case Conference and Discharge
- Prepares and consolidates information ahead of case conference
- Prepares supporting documents for referral to step down care
- Prepares documents for seeking financial assistance for home equipping as recommended by therapist where required
- Means Test
- Check completeness for documentary support for means test/financial assistance review
- Prints and file all documents, information support and reports
- Follows up with families who have not submitted documents
- Contacts families where required to update on timely means test renewal before expiry date, and updates Medical Social Worker/Senior Medical Social Worker on timely need for review of financial assistance validity date
- Medifund/Financial Assistance Scheme
- Preparation and checking accounts receivable status
- Printing of invoices
- Check completeness to listing
- Identify cases to put up for Medifund/financial assistance scheme
- Prepare Medifund/financial assistance scheme forms
- Others
- Assists in tracking department KPIs
- Undertakes any other duties as assigned by Supervisor or Head of Centre

**Job Requirements**
- Recognised Diploma in Social Work or training related to psychology or any related discipline
- Has an interest to pursue or in the midst of pursuing a Degree or Graduate Diploma in Social Work
