**Key Position Objectives**:

- Responsible for conducting/renewing means tests and case management..
- Conduct initial screening and intake assessment for new referrals and manage care plans for the residents.
- Communicate and work closely with family members or caregivers for the residents’ basic needs and care plan. Develop individual care plans and carry out regular review of the cases.
- Liaise with government agencies and other community partners relating to the residents’ care plans.
- Handle assigned administrative duties & reports.
- Attending phone calls, visits and filing.
- Carry out any other duties as assigned by the supervisor.

Assist the Senior Social Worker under the social work team for supporting the residents staying in the Red Cross Home for the Disabled (RCHD).
- Minimum Diploma in Social Sciences (Social Work) or WSQ Diploma in Community and Social Services
- At least 2 years working experience in social service sector
- Good listening and communication skills
- Basic case management skills
- Empathy for persons with disabilities
- Patient in handling family members/caregivers.
- Detailed and meticulous with administrative work.
- Willingness to learn
- Team-spirit
