Building on more than four decades of experience and expertise, NTUC Health is among the largest senior day care, nursing home and home personal care providers in Singapore. It also offers other services for seniors such as an active ageing hub, senior activity centres, community support for vulnerable seniors, and a sheltered / senior group home. In addition, it runs a chain of dental clinics, and a family medicine clinic.
**RESPONSIBILITIES**
The Doctor is responsible for the clients’ care under the Home Care services under NTUC Health, by working closely together with a multidisciplinary team which consists of care professionals like care support staff, nurses and allied health professionals.
- Practice at all times in accordance with the SMC Physician's pledge and abide by the SMC Ethical Code of Conduct
- Work together with a multidisciplinary team to develop care plans for clients, periodically review them and assure that the goals for each care plan are rational and relevant
- Deliver patient centered medical care in a manner consistent with the patient's individual care goals
- Conduct rounds at clients’ homes to ensure care plans are being carried out
- Conduct regular check-ups and diagnosis of ailments, prescribe medication and refer client(s) for further specialist check-ups where required
- Respond in a timely fashion to the client's change in function or condition
- Ensuring patients receive their test results in a timely fashion
- Ensuring proper management of patients' follow ups and return visits
- Communicating with other doctors when referrals are necessary
- Keep abreast of medical information and developments
- Keep abreast of the group's medical practice
- Be an active and contributing member of the Homecare team
**QUALIFICATIONS**:
- Basic medical degree recognized by Singapore Medical Council.
- Registered with Singapore Medical Council
- Relevant experience working with elderly patients, or those with Graduate Diploma in Geriatric Medicine / Family Medicine will be advantageous
- Possess good communication and interpersonal skills
- Willing to travel islandwide to clients’ homes to carry out service
