DESIGNATION : Occupational Therapist (Day Rehab)

RESPONSIBILITIES
- Conduct initial assessment / follow-up reviews / other specific assessments of designated patients in day rehab and day care.
- Develop individual care plans based on patients' needs, adopting a multi-disciplinary team-based approach and implement appropriate interventions / activities.
- Review and re-assess individual care plans regularly and/or when there is a change in patient's condition for both day rehab and day care clients.
- Attend multi-disciplinary round and present cases for discussion / sharing when necessary.
- Make referrals to other service providers, when required and as agreed by the patient or next-of-kin.
- Provide certification on patients' fitness for rehab for rehab services for self-referral and walk-in clients.
- Support HNF's initiatives/projects related to Therapy services.
- Provide occupational therapy treatment for clients at Senior Care Centre.
- Document all inputs in a timely manner.
- Plan and conduct of purposeful activities for clients at the senior care centre.
- Develop Care staff and Therapy associates' abilities in conducting suitable exercises and engaging activities.
- Any other related duties as directed by the said reporting officer.

QUALIFICATIONS
- Minimum Bachelor's degree in Occupational Therapy with at least 2 years' experience
- To be registered with the Allied Health Professions Council in Singapore as Full Registrant
- To have a valid BCLS certification
