
Case Manager, Ambulatory Operations

Case Managment Officer (Renal) is responsible for the administration of the renal programme which provides disease specific management for patients with chronic kidney disease (CKD Stage 1 to 5). He/she should achieve a high standard of customer service, support and to coordinate the administrative and clinical care for renal patients in the inpatient and outpatient setting.
- Initiate, administer and implement programme in line with hospital practices, policies and protocols
- To be liaison personnel working closely with clinicians, nurses, Medical Social Workers (MSWs), and internal and external service providers to ensure smooth operational and clinical aspect of programme
- Provide renal counselling and case management for early and advanced chronic kidney disease (CKD) patients
- Coordinate CKD and pre-dialysis education workshops and relevant training sessions
- Coordinate outpatient vascular access creation appointments, peritoneal dialysis (PD) catheter insertion/removal and training
- Monitor and track patient’s clinical status to ensure smooth transition during conversion from haemodialysis (HD) to PD and vice-versa
- Facilitate interim haemodialysis (HD) placement at external Dialysis Centre (DC) by working with external DC, patients/caregivers and MSWs
- Assist patients’ referrals and coordinate administrative matters for renal transplantation
- Obtain the necessary clinical and administrative information and monitor outcomes/key performance indicators for audit and continuous quality improvement
- To initiate Advance Care Planning (ACP) discussion for pre-dialysis patients in the inpatient and outpatient setting
- Review and ensure policies and procedures pertaining to the programme are documented, established and updated
- Perform any other operational, administrative and ancillary duties as assigned by the reporting officer (e.g. minutes writing)
Requirement
- Bachelor Degree in Nursing, Biomedical Science or Science related.
- At least 2 years prior and relevant experience in case management.
- Able to resolve problems in all aspects of the implementation of programmes including operational, IT, clinical related issues.
