Community Eldercare Manager

1.Organize and plan home visitation to our Eldercare clients;
2. To develop and grow Community Eldercare Services.
3. Assess clients’ needs and raise referrals for clients to appropriate social services provider.
4. Follow-up with and monitor feedback of clients, to ensure recommended services are in place and aligned with
the needs of clients, families and caregivers.
5. Organize recreational activities for the clients.
6. Participate in regular clients review discussions with Community Eldercare Executive and recommend
modification as needed.
7. Provide fast response to clients’ needs.
8. Ensure timely and accurate reporting to the authority.
9. Maintain accurate and timely documentation of clients information, and ensure relevant agencies are kept
updated of changing client needs.
10.Manage a team members and be a role model.
11. Other duties as assigned by management,
**Requirements**:
1. Degree in healthcare and social services fields. Diploma holder with the right experiences will be considered
2. Be prepared to be available for activities during the non-office hours.
3. Team player in a multi-disciplinary team setting with ability to perform basic social screening
4. Strong interpersonal and people skills
5. Passion for working with the elderly and conducting home visits
