DESIGNATION : Senior Associate, Community Connector, Community of Care (CoC)

RESPONSIBILITIES

NTUC Health is one of the largest community health and eldercare providers in Singapore. Through our Active Ageing Centres, we engage the seniors within the clusters and support them by reaching out to understand their needs, curating programmes that cater to the needs of active seniors, and befriending isolated and vulnerable seniors.

COC 2.0 involves the identification, planning and implementation of integrated community care ecosystems to support ageing in place. This role supports the execution of project Community of Care (COC 2.0) - Go Broader. The position will be based at either AAC-Bukit Merah View or Jurong Central Plaza.

**Partnerships Engagement**
- Support the implementation of the new playbook / workflows, document processes, learnings, challenges and solutions, as well as implement any needed improvements for long-term sustainability
- Stocktake and assess the strengths and limitations of existing partnerships and processes, review and optimise existing workflows
- Assist in the development and coordination of the engagement efforts with partners
- Develop an asset map of community partners and resources to identify gaps in existing services and strengths for integrated care partnership engagement

**Reporting**
- Work with established partners and government agencies to collect and report relevant data for analysis and evaluation, as well as document learnings, challenges and best practices from the pilots
- Assist with the preparation of reports to funders and management and ensure compliance to reporting requirements

**Data Management and Needs Analysis**
- Support the Active Ageing Centres (AACs) in data analysis to identify needs of the community
- Support data collection, analysis and evaluation
- Maintain a database of reports, data, and documents related to CoC efforts
- Compile learnings, challenges, and best practices from pilots and partnerships

Any other duties as assigned by reporting officer

QUALIFICATIONS
- Diploma in Social Work, Community Development, Business, Marketing, Mass Communications preferred
- At least 2 years experience in project management, business development or relationship/stakeholder engagement
- Proficient in Google Suite Applications
- Commitment to the organisation's mission, vision, and values, which includes an understanding of and commitment to improve ageing issues in Singapore
- Ability to engage multiple stakeholders whose needs and experiences vary in support of a shared goal
- Excellent written and verbal communication skills, formal presentation skills, organisation and planning skills
- Able to work independently and at the same time a team player who can work cohesively with colleagues from different levels
- Proactive and highly accountable for the work assigned

OTHER INFORMATION
- Knowledge in using data-driven approach to identify problems and implement innovative solutions
- Proficiency in data collection, analysis and reporting
- Has an interest/passion for working with seniors and people with diverse backgrounds and experience

**The employee may be required to work on some weekends and weekday evenings when there are programmes or events during those times.**
