Social Worker Geriatric Care

Assess the community health and social care needs
- Coordinate support and service placement (where required)
- Develop and implement calendar plan of social and health activities for seniors
- Monitor service users' physical/emotional/social need and conduct care assessment
- Individualised Care Plan for each service user with relevant stakeholder and encourange to participate in programmes and activities
- Participate in case management discussions with external service providers
- Perform community profiling and identify the needs, interests and capabilities of residents
- Monitor and statistics and records of the services rendered
- Liaise with outside agencies on matters related to the service users
- Mentor staffs/volunteers and participate in staff development programme

**Requirements**:

- Singaporean Only
- Diploma/Degree in Social Work
- Able to assist with Basic translation from English to Mandarin
- Full Time Position; Mon to Fri 9am - 6pm
