**JOB SUMMARY**
Working as part of a multi-disciplinary team, you will take on the role of Care Co-ordinator between patients, medical specialists and TTSH’s community partners.
- This is a para-clinical / nursing admin role, where you will conduct the following duties that include but are not limited to clinical screening and identification of patients based on set criteria, case management of complex cases with Diabetes, database management and liaison with clinical care partners in TTSH or in the community e.g. General Practitioners/ Polyclinics. In addition, you will also assist in any other duties as assigned by the Supervisor / Nursing Manager.
- This position will be physically based at the TTSH AMK Specialist Centre located at 723 Ang Mo Kio Avenue 8, Singapore 560723; with occasional travelling (reimbursement provided) to the main campus at TTSH when required. Hours can be flexible, as arranged with the Supervisor.**MAIN DUTIES AND RESPONSIBILITIES**
- Screen & identify new cases based on established criteria
- Coordinate the transfer of care of patients between AMKSC specialists and community partners such as General Practitioners or Polyclinics
- Serve as main liaison point between physician, patient and community care providers
- Facilitate multiple care aspects (e.g. case coordination, information sharing, record patient follow up plans, indicators tracking etc.)
- Patient education and outreach to community care partners
**JOB REQUIREMENTS**
- Diploma OR Bachelor Degree (e.g. Nursing or healthcare-related fields)
- Relevant Diploma at least 4 - 5 years of nursing or healthcare experience OR Bachelor Degree with at least 1 - 2 years of healthcare experience
- Affable personality with ability to work in a team and build relationships with a diverse group of stakeholders
- Previous experience in nursing management of Diabetes is an advantage but not a pre-requisite
- Good communication skills, and ability to engage stakeholders at all levels
