**RESPONSIBILITIES**

As a Social Work Assistant, you are required to provide administrative support to the Social Work team in areas related to referral management, casework follow-up, and service coordination for clients at the day centres.
- Receive and create referral records for all day centre referrals
- Supports Social Worker/Caseworker in intake processes, coordinating admissions, service plans, and providing casework follow-up
- Assist with data entry and maintenance of client's case records and reports for audit and compliance purposes
- Compile and report statistics to various stakeholders such as MOM and Agency for Integrated Care (AIC) on a monthly basis or as necessary
- Coordinate and set up Multidisciplinary Team Meetings (MDM), including scheduling, sending reminders, and taking minutes when required
- Attend to public and caregiver enquiries
- Assist Social Worker/Caseworker to coordinate family care conferences, home assessments and visits when necessary
- Provide administrative support

**Job Requirements**
- At least 1 year of relevant experience in administrative roles; experience in the social service or healthcare sector is an advantage
- Proficient in Microsoft Office (Word, Excel) and comfortable using IT database systems
- Strong organizational skills and attention to detail
- Good communication and interpersonal skills, with excellent telephone etiquette
- A team player who is patient, warm, helpful, and well-organized
- Willing to travel between centres

**QUALIFICATIONS**
- O/A Levels or Diploma in Social Work/Community Service/Human Services or an equivalent field

**Working Hours**

Under normal circumstances the hours of duty will be as follows:

- 8.30am to 5.45pm (Monday to Friday, 45 minutes lunch break)
