- **Provide case management and appropriate level of care through risk assessment, care planning, and interventions to youths at risk and their families.**:

- **Collaborate with schools and community stakeholders to implement a holistic intervention and supportive care plan for youths and families.**:

- **Conduct and facilitate psycho-education group sessions according to the curriculum provided to youths.**

- **Relevant work experience of at least 2 years with youths and families is preferred.**:

- **Preferably trained in FAST (Family and Adult Support Tool) and/ or YLS/CMI (Youth Level of Service/ Case Management Inventory).**:

- **Knowledge and ability to case assess, engage, and carry out intervention plans in a timely manner.**:

- **Be able to work with clients in time-based programs to achieve desired outcomes.**:

- **Good team player with interpersonal skills in working with youth.
