Job Opportunity:

Key Responsibilities:

Engage residents and significant others in a therapeutic manner to understand their needs and underlying issues. Conduct thorough assessments, integrating information from relevant stakeholders, and initiate inter-agency conferences for complex cases. Apply advanced clinical and professional theories to develop and execute social work intervention plans across the resident's trajectory of illness. Manage and overcome reluctance and resistance to change, helping residents/significant others to develop and choose strategies and formulate viable plans. Follow up and review residents' changing needs and update care plans to ensure relevance and effectiveness of intervention. Conduct field visits as required. Develop protocols and work with residents, families, multi-disciplinary teams, and stakeholders to arrange for holistic and resident-focused care plans. Lead multi-disciplinary team or inter-agency case conferences to formulate holistic care plans, advocate for resident dignity, confidentiality, and rights, and advocate for quality of life improvement. Provide professional advice or clinical intervention in complex cases, including crisis management (suicide or abuse) for residents/significant others and staff. Document assessments and interventions in progress notes. Prepare reports upon
