Senior / Staff Nurse - Outpatient (Diabetes Care)

Location: West (5 days, Clinic Hours)Role Overview: Provide nursing support in a specialized outpatient service focused on diabetes management. Responsibilities include: Assess patients’ lifestyle, diet, and social factors to provide personalized coaching to better manage diabetes. Collaborate closely with physicians to develop and implement individualized care plans. Educate patients on insulin use, blood sugar monitoring (including technology-assisted methods), and interpreting glucose data, both in outpatient and inpatient settings. Support quality improvement projects within the clinic to enhance patient outcomes. Perform other tasks as assigned. Qualifications: Registered Nurse with recognized diploma or equivalent. Specialist certification in diabetes education preferred but not required. Minimum 1 year experience in chronic disease or diabetes management. We are seeking a qualified candidate for this role.
