Job Summary
We are seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will be responsible for providing exceptional nursing care to elderly residents in our centre or residence. Your primary goal will be to deliver optimal care while promoting independence and dignity for our residents.

Key Responsibilities

  • Provide nursing leadership and ensure the delivery of high-quality care to all residents.
  • Orient and guide junior nursing personnel through continuing education and supervision to achieve quality care.
  • Develop and implement individualized care plans that cater to the unique needs and preferences of each resident.
  • Conduct daily rounds to assess and monitor the health and nutritional status of residents, identifying potential issues and implementing interventions as necessary.
  • Administer prescribed medications according to established protocols and guidelines.
  • Monitor and report adverse reactions and effectiveness of medications.
  • Evaluate the effectiveness of interventions and make recommendations for improvement.

Requirements

  • Diploma, Advanced Diploma, Higher Diploma, or Graduate Diploma in Nursing or a related field.
  • A minimum of 3 years of experience in the related field, with a proven track record of delivering high-quality care.
  • Valid Singapore Nursing Board License (Staff Nurse).
  • Excellent communication, interpersonal, and organizational skills.
  • The ability to work independently and under pressure, prioritizing tasks and managing time effectively.
