Elderly Care Coordinator

About the Role:
We are seeking a dedicated and compassionate Elderly Care Coordinator to join our team. As an Elderly Care Coordinator, you will be responsible for providing quality care and companionship to a group of elderly clients assigned to your care. Your primary goal will be to ensure that each client receives the highest level of care and support to enable them to maintain their independence and dignity.

Responsibilities:
  • Liaise with clients and their families to understand their needs and preferences
  • Develop and implement individualized care plans to meet the unique needs of each client
  • Provide personal care and hygiene services, including assistance with daily living activities such as bathing, dressing, and toileting
  • Administer medication reminders and oral feeding as needed
  • Lead and participate in group activities designed to promote socialization and mental stimulation

Requirements:
To be successful in this role, you will need to have excellent communication and interpersonal skills, as well as a compassionate and empathetic approach to caring for others. You should also be able to work independently and as part of a team, and be comfortable working in a fast-paced environment.
