Support the admission and discharge process for residents. Keep accurate records of residents and referral updates. Help review referrals, do means testing, manage cases, and communicate with residents and families. Do regular follow-ups to make sure residents continue to receive proper care. Conduct home visits and support activities like family conferences, case conferences, and care projects. Encourage family involvement and regular visits. Coordinate volunteer activities and programmes. Arrange timely and appropriate service referrals for seniors. Help out with projects and programmes when needed. Attend required training and social work supervision sessions. Service Operations Work closely with the multi-disciplinary team to deliver person-centred assessment, care planning, and discharge processes Collaborate with residents and families to improve quality of life, independence, and social integration Assist multi-disciplinary teams in service quality and improvement initiatives Collate and analyse trends and needs of vulnerable groups Others Prepare weekly, monthly, and ad-hoc management reports Perform other duties as assigned by the Manager, Medical Social Work or Executive Director Working Hours 5-day work week, Monday to Friday, 8.30am to 6.00pm Some Saturdays required for events involving residents Thank you for applying.
