Job Opportunity:

Social Worker/ Community Care Worker is a vital role that enables you to make a significant difference in the lives of our clients. As a professional in this field, your primary responsibilities will include building rapport with residents/clients and their families, maintaining constant communication, and addressing their concerns and goals.

  • Develop strong relationships with residents/clients and their families through active listening and empathetic communication.
  • Collaborate with external agencies such as hospitals and polyclinic to provide comprehensive support services for clients who require financial assistance for medical appointments.
  • Design and implement individual care plans (ICP) tailored to the unique needs of each client, providing therapeutic support as needed.
  • Deliver effective casework management, monitoring progress, and developing programs and activities through a structured approach.
  • Conduct thorough financial assessments, manage funds, and prepare periodic reports as required, identifying and referring cases for financial assistance.

Requirements:

To excel in this role, you must possess the following qualifications:

  • A Degree in Social Work.
  • At least 2 years' experience in social work.
  • The ability to work effectively with diverse groups, including the elderly and various stakeholders.
