Community Support Specialist

Key Responsibilities:
- Develop and implement individualized case management plans to meet the diverse needs of residents.
- Collaborate closely with Probation Officers, Child Protection Officers, and other stakeholders to ensure effective co-case management and achieve positive outcomes for residents.
- Forge partnerships with employers, schools, and community agencies to facilitate reintegration efforts and promote sustainable support systems for residents.
- Present cases at various forums to keep stakeholders informed about progress and address any emerging concerns.
- Maintain accurate and up-to-date casework documentation to ensure seamless communication and collaboration among team members.
- Coordinate with Hostel Operations to provide essential information for daily movement and ensure smooth operations.
- Conduct group sessions for residents to foster a sense of community and shared purpose.
Requirements:
- A degree in Social Work or equivalent qualification, coupled with 2 years of experience working within the social service sector.
