Senior Care Nurse Position

We are seeking a highly skilled and compassionate Senior Care Nurse to join our team. As a key member of our interdisciplinary care team, you will be responsible for providing exceptional care to seniors in their homes.

Key Responsibilities:

  • Conduct regular home visits to assess the needs of frail and home-bound seniors.
  • Triage client conditions and respond promptly to address any issues.
  • Develop and implement personalized care plans in collaboration with our team.
  • Communicate care goals and plans to clients and caregivers to foster active participation.
  • Evaluate the effectiveness of interventions and revise care plans as needed.
  • Manage and monitor clients' medical and nursing needs.
  • Maintain accurate documentation of client progress and outcomes.
  • Collaborate with healthcare professionals to ensure seamless care delivery.

Requirements:

  • Professional Certificate or Diploma in Nursing, preferably with specialization in gerontological nursing.
  • State registration with the Singapore Nursing Board (SNB) and a valid Practising Certificate.
  • 2-3 years of experience in the social services or eldercare sector.
  • Extensive clinical knowledge and skills.
  • Ability to work effectively with seniors and caregivers.
  • Flexible, energetic, and able to multitask.
  • Strong communication and interpersonal skills.
  • Bilingual in at least two local dialects to facilitate communication with seniors.
