Job Scope: Facilitate structured and empathetic Advance Care Planning conversations with patients and their families. Educate individuals on goals-of-care, advance directives, and other end-of-life care options. Work collaboratively with an interdisciplinary healthcare team to ensure ACP documents are accurately integrated into patient records and effectively communicated. Actively contribute to the development and rollout of ACP programs and initiatives within the organization. Uphold the highest standards of confidentiality, ethics, and organizational policy in all interactions.
