The Opportunity: We are looking for Senior / Staff Nurse (SOC- Diabetes Clinic to join us to provide high quality patient care. Job Responsibilities: The incumbent shall be responsible to: Assess patients' social history, lifestyle, and dietary habits to provide tailored counselling and help patients better understand and manage their diabetes. Collaborate closely with the Endocrinologist to develop personalized care plans for effective diabetes management. Conduct comprehensive patient education on insulin injection techniques, glucose monitoring (via glucometer or continuous glucose monitoring), review of glucose profiles, and other diabetes-related counseling in both inpatient and outpatient settings. Support the clinic's quality improvement initiatives to achieve better clinical and process outcomes. Perform any other duties as assigned by the Reporting Officer. Job Requirements: Diploma in Nursing or State Registered Nurse Certificate. Specialist Diploma in Diabetes Education or a Certified Diabetes Educator certification preferred. Minimum 2 years of relevant experience, with a focus on chronic disease management. Demonstrated experience in providing comprehensive health education to patients with diabetes. Excellent clinical judgment, critical thinking, and problem-solving skills to deliver patient care in line with established protocols. Proactive attitude, strong team spirit, and professional maturity.
