Senior/ Manager, Regional Health Community Outreach - West

To support the Board's goals, the Regional Health Systems and Community Outreach Division collaborates closely with the community and healthcare partners to co-create regional and community-owned solutions to ensure good coverage and sustainable delivery of priority health promotion programmes in the community setting. Key programme areas include active ageing, physical activity, nutrition, substance abuse, and mental health.
This person will partner with community stakeholders to identify opportunities for implementing and supporting initiatives for a healthier living environment, especially for underprivileged families with young children. He/she will also build partnerships and link up partners into healthy ecosystems to facilitate the integration of preventive health bundle programmes in their communities.
SCOPE OF WORK WILL INCLUDE BUT NOT LIMITED TO:
Plan, implement, and champion sustainable Healthy Community Ecosystems (HCE) for the population under the assigned region to amplify our health promotion efforts.
Collaborate with key partners from government and non-governmental organisations, Regional Health Systems, community partners, volunteers, and vendors to increase outreach and strengthen the quality of programmes/services offered in the community setting.
Work closely with internal HPB divisions to create and deliver programme packages suitable for your region.
Manage and monitor day-to-day operations of health promotion programmes delivered by outsourced vendors to ensure smooth implementation in the community.
Monitor targets through data analysis and develop action plans to achieve them with prudent management of programme budget.
Prepare and update progress reports for various projects to the Division, Senior Management, and internal and external stakeholders.
REQUIREMENTS AND QUALIFICATIONS
Previous working experience as an Outreach and Partnerships Manager for at least 3 years.
A very good understanding of partnership development and relationship management, with the ability to network easily and build strong collaborations with stakeholders.
Excellent communication and interpersonal skills, with versatility in managing different levels of partnerships.
Strong organisational and project management skills; good multitasking skills with attention to detail.
Ability to work independently and with teams.
Prior experience and knowledge in social service settings will be an advantage.
Passion to serve, particularly in the community setting.
