Patient Population: Paediatric patients for renal transplantation
Specific Roles and Responsibilities
Plan and conduct personalized patient and family education for pre- and post-renal transplant patients.
Serve as a liaison with the adult transplant coordinator for paediatric renal transplant patients.
Respond to and address immediate queries/concerns from renal transplant patients or their families related to child's condition and pre/post-transplant management.
Assist in monitoring new and long-term post-transplant patients.
Assist in kidney biopsies.
Monitor primary patients' progress and compliance with medication.
Serve as a resource person for renal transplant patients and families requiring phone consultation services.
Recommend appropriate referrals to allied healthcare professionals.
Plan and conduct staff education on paediatric renal transplant for nursing staff and clinical fellows/doctors.
Serve as a resource for nursing staff, nursing students, and other healthcare personnel in acquiring knowledge and skills related to renal transplant.
Participate in dialysis and transplant clinics, paediatric renal replacement programme meetings, and the annual Children's Kidney Centre camp.
Update and maintain transplant administrative activities.
Document relevant registry statistics for transplant according to Ministry of Health requirements.
Assist in writing transplant protocols.
Develop, review, and update educational materials for patients and families with renal diseases.
Coordinate activities and act as a link with established support groups like Club Rainbow for children and their families.
Provide coverage for urgent nursing duties under KTP NUCMI and take on additional responsibilities or tasks assigned by the senior nurse, manager, clinician, educator, or assistant director of nursing.
