Geriatric Care Specialist Role

We are seeking a skilled professional to join our team in delivering high-quality geriatric care services.

  • Create personalized care plans tailored to the needs of residents.
  • Implement care plans and regularly record residents' progress, ensuring optimal outcomes.
  • Design and organize programs and activities to address residents' care requirements, enhancing their overall well-being.
  • Prepare and submit review reports and monthly interview records every six months, facilitating continuous improvement.
  • Collaborate with relevant stakeholders to coordinate and ensure comprehensive care for residents, promoting a seamless experience.

Required Skills and Qualifications:

  • Diploma/Degree in Social Work/Psychology/Counselling.
  • At least one year of relevant healthcare industry experience, with a proven track record of success.
  • Good interpersonal and problem-solving skills, with the ability to work independently and as part of a team.
  • Proficient in English and able to converse in dialects or other languages, facilitating effective communication.
