Staff Nurse Job

We are seeking a skilled and compassionate staff nurse to provide high-quality nursing care to frail and home-bound elders in their homes.

Main Responsibilities:
  • Conduct regular home visits to assess the health and well-being of clients
  • Triage client conditions and respond promptly to address any issues that may arise
  • Develop and implement individualized care plans with the interdisciplinary team
  • Educate clients and caregivers on care goals and care plans to ensure active participation
  • Monitor and evaluate client outcomes, revising care plans as needed
  • Manage client medical and nursing needs, maintaining accurate documentation of progress and outcomes
  • Collaborate with doctors and other healthcare professionals to provide comprehensive care
  • Assess and provide education and training for clients, caregivers, and team members
Requirements:
  • A professional Certificate/Specialist Diploma/Advanced Diploma in Nursing, preferably with gerontological nursing experience
  • State registration with the Singapore Nursing Board and a valid Practising Certificate
  • At least 2-3 years' experience in the social services or eldercare sector
  • Extensive clinical knowledge and skills
  • Familiarity with working with the elderly and caregivers
  • Ability to multitask, work flexibly, and possess excellent communication and interpersonal skills
