Job Summary

We are seeking a dedicated and compassionate Staff Nurse to join our Home Care team. The successful candidate will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities:
  • Participate in a pilot home palliative project, delivering hands-on bedside care to patients in need.
  • Take on respite assignments as required, ensuring continuity of care for our clients.
  • Collaborate with the home care team on various nursing cases, providing expert guidance and support.
  • Advise care staff on medical issues or emergencies, promoting a safe and healthy environment for our clients.
  • Conduct hospital or home visits to assess new clients for home care services, identifying their unique needs and requirements.
  • Work closely with the team to manage complaints and crisis situations, resolving issues promptly and professionally.
  • Develop strong relationships with external community partners and colleagues, ensuring seamless coordination of client care.
  • Maintain accurate and comprehensive documentation, adhering to MOH guidelines for financial and quality audits.
  • Perform other duties as assigned by management, contributing to the overall success of our team.

Requirements:
  • Minimum Diploma or Bachelor's Degree in Nursing, with a strong academic record.
  • Registered Nurse with Singapore Nursing Board, holding a clean disciplinary record.
  • Proficiency in Microsoft Office and familiarity with IT systems, used regularly during work hours.
  • Occasional shift work may be required, particularly for palliative patients requiring around-the-clock care.
