This is a dynamic role for a motivated and detail-oriented Renal Coordinator to join our team. The ideal candidate will have excellent communication skills, be able to work independently and as part of a team, and have a strong passion for delivering exceptional patient care.
Job Responsibilities:
- Patient Care:
- Provide case management services for patients with early chronic kidney disease (CKD) to delay disease progression through education, follow-up appointments, and evaluation of investigation results.
- Manage advanced CKD patients and their family members for smooth transition from non-dialysis dependent to dialysis dependent through pre-dialysis counseling, coordination of access creation, follow-up on patient decisions regarding modality, and application/approval of Voluntary Welfare Organisation (VWO) and interim haemodialysis (HD) centre arrangements.
- Liaise with vendors, dialysis centres, patients, and caregivers pertaining to end-stage kidney disease.
- Conduct Peritoneal Dialysis (PD) competency for patients deciding on PD.
- Conduct Advance Care Plan (ACP) discussions for pre- and post-initiation dialysis patients in the inpatient and outpatient setting.
- Follow up on patient care, including hepatitis B vaccination, anaemia management, calcium phosphate control, and nutrition status.
- Monitor and track statistics on workload and quality outcomes.
- Track patients with dialysis-related issues such as adherence to treatment, cardiac complications, anaemia, and nutrition, especially those under renal transplant registry and waitlist.
- Coordinate the care of patients under renal transplant waitlist and requiring yearly review at transplant centre in tertiary hospital.
- Monitor outcomes of inpatient dialysis-related complications and hospitalisations.
- Collaborate with nephrologists, surgeons, dialysis nurses, medical social workers, dietitians, and pharmacists to ensure quality patient care.
- Arrange and coordinate seminars/workshops on kidney disease.
- Complete monthly reports, track workload, key performance indicators, and balance scorecards.
- File and handle patient documents systematically, orderly, and confidentially for audit preparation.
- Submit monthly HD and PD patient statistics to National Registry Disease Office (NRDO).
- Initiate self-growth and development through on-the-job training.
- Ensure confidential information is kept safe at all times.
- Identify problem areas, recommend, and implement improvements.
- Perform administrative and data collection tasks.
- Undertake other ancillary duties assigned by Nephrologist/DOA/HOD/COO.
- Embody continuous improvement as a way of life by refining work performance daily.
Required Skills and Qualifications:
- Bachelor's Degree in Nursing.
- 2 years working experience, preferably with renal-related experience.
- Basic IT knowledge required; good PC knowledge an advantage.
- Positive attitude and consistent good performance.
- Good interpersonal and communication skills.
- Positive attitude, initiative, and keenness to learn.
- Able to work independently.
- Proactive and able to implement change.
Professional Licence:
- Singapore Nursing Board (SNB) licence.
