Job Title: Holistic Health Facilitator

Description

The incumbent plays a pivotal role in providing comprehensive health solutions to patients beyond rehabilitation at our centers.

  • Identify, promote, and implement holistic wellness programs for the community dialysis center.
  • Manage screenings, physical assessments, data compilation, analysis, and recommendations for follow-ups for dialysis patients.
  • Conduct regular reviews to monitor patients' compliance and progression in exercise programs and ensure care plans are followed through.
  • Maintain exercise and program equipment, ensuring regular disinfection and servicing.
  • Manage patients' attendance to ensure accurate and timely documentation, maintaining consistent participation.
  • Collaborate with the Clinical Services Team to address any barriers to participation and ensure patients receive necessary support to continue their wellness journey.
  • Assist in organizing volunteer events and outings to empower seniors and encourage engagement.
  • Additional responsibilities as required.

Required Skills and Qualifications

  • Minimum Diploma in Health Sciences or relevant courses (e.g., sports sciences, therapy aide, community care associate).
  • Relevant working experience in a healthcare setting is advantageous.
  • Strong interpersonal skills and proficient in Microsoft Office.
  • Positive and committed attitude with a strong desire to serve the community.
  • Highly organized, detail-oriented, and multitasking ability.
