Job Title: Community Care Associate

We are seeking a compassionate and dedicated Community Care Associate to join our team at REN CI HOSPITAL. As a Community Care Associate, you will play a vital role in providing high-quality care and support to our clients, ensuring their physical, emotional, and social well-being.

Key Responsibilities:
  • Facilitate and support clients' activities of daily living (ADLs), taking into consideration individual preferences.
  • Assist nurses or therapists in providing routine care and improving clients' quality of life.
  • Support planning and implementation of daily programs at the day care.
  • Perform daily work duties according to department or organization standard operating procedures (SOP).
Care Delivery:
  • Prepare and serve meals and tea breaks.
  • Assist clients with oral feeding.
  • Support clients with personal hygiene activities, such as toileting, diaper changing, and grooming.
  • Deliver NGT/PEG feeding.
  • Assist with medication administration.
  • Monitor and record vital signs, reporting any abnormalities to staff nurses or therapists.
  • Conduct cognitive and behavioral monitoring and interventions.
  • Provide simple wound care management under the supervision of staff nurses.
Programme Planning and Implementation:
  • Support the implementation of organization-wide programs, such as volunteer activities and fundraising events.
  • Contribute ideas and support the implementation of site-specific programs, such as daily day care activities and community outings, in collaboration with admin assistants and therapists.
Incident Reporting:
  • Report and escalate any client or staff-related adverse incidents, such as falls, injuries, pressure ulcers, and medication errors.
  • Support the investigation of incidents.
Caregiver, Next of Kin (NOK), and Client Satisfaction:
  • Escalate complaints or disputes from caregivers, NOK, and clients to the Reporting Officer (RO).
Administrative Duties:
  • Responsible for housekeeping, maintaining, and cleaning equipment at the Senior Care Centre (SCC).
  • Purchase materials required for activities and submit claims for day-to-day purchases.
  • Support inventory monitoring.
  • Perform other duties as assigned by the RO.
Professional Development:
  • Participate in relevant training programs to acquire skills and knowledge that enhance work performance.
Academic Qualification:
  • Minimum GCE 'O' or 'N' level.
  • Relevant skills certificate from a recognized institution will be an advantage but not essential.
Experience:
  • Working in a geriatric setup, long-term care, or nursing home setup will be an advantage but not necessary.
  • Mature and a good team player.
  • Able to multitask and work in a stressful environment.
  • Able to maintain the honor, interest, and dignity of the institution or organization.
  • Able to serve those in need, regardless of race, nationality, religious, or social status.
  • Compassionate and able to relate well with the sick patients of the organization.
  • Caring, understanding, patient, and empathetic.
  • Cooperate with all colleagues towards the total healthcare of the community.
